Home Health

Home Health Services are delivered by a case management team consisting of a registered nurse and a graduate level social worker to help clients remain independent in their homes.

Clients meet face-to-face with their case management team at least every 60 days and speak via telephone every 30 days to re-evaluate their plan of care. These meetings assist clients in obtaining continuity of care through referrals to services within the community, psychosocial education and advocacy with the client's primary care physician.
